2009 Athletic Award Winners

Pre-Participation Exams
Bellin Health Sports Medicine and Bellin Health Family Medical Centers are offering pre-participation exams for the upcoming 2009-2010 athletic seasons. The exams will be offered for $20 per student athlete. All student-athletes must be accompanied by their parents or have the signed parental consent form at the time of the visits. The WIAA green forms are no longer available from the WIAA office. It is a downloadable form that is on the WIAA website http://www.wiaawi.org/forms/physicalcard.pdf Student-athletes are encouraged to have this form filled out and signed prior to arriving at the time of his or her exam.

Preble student-athletes can utilize the following dates, locations and times:
Green Bay:
July 20 (6-8 p.m.) Bellin Family Medical Center (920-430-4750)
July 22 (6-8 p.m.) Bellin Family Medical Center (920-430-4750)
July 29 (6-8 p.m.) Bellin Family Medical Center (920-430-4750)
August 12 (6-8 p.m.) Bellin Family Medical Center (920-430-4750)
